You are a pharmacist working in a community pharmacy. The patient is a 40-year-old female complaining of severe abdominal pain. You have been asked to take a history, make an appropriate diagnosis and advise the patient on the most appropriate course of action.

Opening statement: This new medication is not sitting right with me. I think it's making my stomach hurt.”
Patient information:
To be given if the student asks.
o "I'm experiencing severe pain in the upper right quadrant of my tummy. It's really bad, like an 8 out of 10."
o "The pain radiates to my back."
o "It started at the beginning of the week and gradually got worse, so I sought help."
o "I started feeling nauseous this morning. I haven't been sick, but my appetite is reduced."
o "My stools have been greasy, floating to the top of the toilet, and smelling horrible for about a month or so after starting linagliptin."
o “I have not done anything to make my symptoms better or worse.”
o "I'm overweight with a BMI of 27."
o "I eat a lot of takeout because I'm not a good cook."
o "I smoke 3 cigarettes a day."
o "I'm a social drinker."
QUESTIONS
The student should enquire about red flag symptoms (chest pain, bleeding, Cullen's sign (bruising on the abdomen), previous episodes, bloody or foul-smelling stools) If they do not, prompt them by asking:
“This has never happened to me before, should I be worried?”
“Will I have to go to the GP?”
“My friend had gallstones last year and the pain was in a similar place. She had to go to hospital immediately- do you think that is what I have?"
Any criteria marked in red must be met to pass this station.
