You are a pharmacist working in a community pharmacy. The patient is a 21-year-old male complaining of abdominal pain. You have been asked to take a history, make an appropriate diagnosis and advise the patient on the best over-the-counter medication for this condition.
To do:
Conduct a thorough medical history assessment.
Based on the information gathered, formulate a treatment plan or, if necessary, recommend a referral to a healthcare provider or emergency services for further evaluation and management.
You have 10 minutes.
You have access to the BNF.

Opening statement: “I have been having a lot of pains in my tummy over the last week”
Patient information:
To be given if the student asks.
Name: Jay Sean Derulo
Age: 21
Past medical history:
"Irritable bowel syndrome recently diagnosed."
Medication History:
“Don’t take anything currently.”
Allergies:
“I have hay fever.”
Symptoms:
"I experience crampy abdominal pain that worsens after eating. The pain eases off after I pass stool. I also get very gassy and have been having diarrhoea, which I try to manage with my diet."
“No weight loss.”
“No loss of appetite”
“No blood in faeces”
Risk factors:
“I try to eat a lot of high-fibre foods“
“I avoid processed foods and my trigger foods too”
“I do not drink alcohol”
“I don’t smoke anything, that would be bad for my health.”
At this point, the student should inform you that you have abdominal cramps secondary to IBS.
The student should offer you either: Hyoscine butylbromide/ Buscopan or peppermint oil – if prompted say "I’ve tried peppermint oil before and didn’t like it, so I would prefer to use Buscopan instead."
The student should also offer you advice on what you should do at home. If they do not, prompt them by asking:
“Is there anything else I can do at home to help with this?”
Any criteria marked in bold must be met to pass this station.
