Accuracy check — what to verify:

• Patient name and DOB — must match exactly between label and prescription

• Drug name, strength, and formulation — must match prescription

• Directions — must match prescription exactly

• Quantity — label must match prescription; physical contents must match label

• Expiry date — must not be passed at the time of dispensing

You are a pharmacy student on placement in a community pharmacy dispensary. The supervising pharmacist has asked you to carry out an accuracy check on a dispensed item before it is handed out to the patient.

You will be given:

• The original FP10 prescription

• A dispensed bag containing one labelled medicine box

To do:

1. Work through the accuracy check systematically, checking the dispensed item against the prescription.

2. Verbalise each check aloud to the assessor as you go

3. State your conclusion clearly

4. If you identify an error, state what the error is and what you would do next.
