In this scenario, a patient visits your pharmacy with a bite. The pharmacist should take the relevant history, diagnose, and suggest an appropriate management plan. There are no images and you are not expected to examine the patient.
Please ensure that this scenario and mark scheme aligns with the most up-to-date guidelines from the UK NICE and the BNF when using it for your OSCE assessment.
You will need a student pharmacist and an actor for this OSCE station.
Pharmacy First Criteria 2023
Infected Insect Bites
You are a community pharmacist. A patient has come to the pharmacy seeking information about their symptoms. You are expected to take a detailed history of the patient, respond to their questions appropriately, ensure patient safety and prescribe the appropriate medication.
To do:
Conduct a thorough medical history assessment.
Based on the information gathered, formulate a treatment plan or, if necessary, recommend a referral to a healthcare provider or emergency services for further evaluation and management.
You have 8 minutes.

“I drink beer every night.”
Symptoms:
“I think I have a bite or sting from an insect on my leg.”
“The skin is swollen.”
Important negative symptoms:
“No difficulty breathing, nausea, abdominal pain, cramping, loss of consciousness.”
“No rash.”
“Not itchy.”
“No discharge.”
“No fever.”
If the pharmacist asks:
“The spot that was left behind is now grown, more red and painful. It is also tender to the touch.”
“I just returned from a getaway from the peak district 4 days ago.We did some hiking and climbing."
“The spot and the skin around it is hot to touch.”
“I can’t show you because of my jeans.”
“The sting is only on the leg.”
“I am not allergic to penicillin.”
The student should at this point tell you that - you have an infected insect bite. They should offer you flucloxacillin for 5 days.
For your pain they should offer you a suitable painkiller and give self-care advice:
“Thank you I will try this.”
Any criteria marked in red must be met to pass this station.
Achieving at least 50% OR missing specific required marks.
