You are a hospital pharmacist. You are asked to review a patient with severe rheumatoid arthritis who has been admitted following a fall. You need to check the drug chart, respond appropriately, and ensure proper communication with the prescriber.
To do:
Conduct legal, clinical and accuracy checks on the drug chart. Find any discrepancies and discuss them with the patient. Provide information on the actions you will be taking if any discrepancies are found.
You have 10 minutes.
