In this scenario, a ward pharmacist takes a medication history from a 62-year-old man admitted with worsening abdominal and ankle swelling, on a background of cirrhosis and type 2 diabetes.
The student must take a complete medication history, including over-the-counter, herbal and borrowed medicines, reconcile what the patient is actually taking against his GP repeat list, identify the medicines contributing to this admission, and explain what they would do about them.
