In this scenario, the student pharmacist is tasked with counselling a patient who is concerned with IBS-like symptoms. The student must provide guidance on lifestyle and treatment options, answering any questions they have.

You will need a student pharmacist and an actor for this OSCE station.
Menopause
You are a pharmacist working in a community pharmacy. A 46-year-old female patient has come to the pharmacy seeking advice on some of her symptoms. She is considering taking supplements but would like to know more about her options.
To do
Explore the patient’s current symptoms.
You are expected to provide advice, counsel this patient and to answer any questions appropriately. You must provide relevant advice.
You have 10 minutes.
You are Sophie Thompson (Soh-fee, Tomp-son)
Opening Statement: “I’ve been having menopausal symptoms for a couple of years and I’ve been managing them with my lifestyle quite well, but I can’t seem to deal with these problems with my tummy.”
Patient Information:
Name: Sophie Thompson
Age: 46
Symptoms:
“I’ve been getting bloating.”
“It's been happening for the last year.”
“I’ve been feeling constipated too. I used to go for a poo every day and now I only go every 3 or 4 days.”
“I haven’t had a period in 5 months.”
“I had issues with sleeping and hot flushes but I managed those with cooling techniques and sleep hygiene.”
No other symptoms
Medical History:
“I have pre-diabetes, so I need to get on top of my diet.”
“No other conditions.”
Medication History:
“I don’t take any medication.”
“I take vitamin D supplements over-the-counter because it was low in a blood test I did once.”
Allergies: “I don’t have any allergies.”
Lifestyle factors:
“I eat a lot of takeaways and processed food. I try to eat fruits and vegetables, but I know I could improve my diet.”
“I don’t drink much water”
“I don’t exercise regularly, but I am looking to start going to the gym.”
“I don’t smoke or drink alcohol.”
“I work as a hairstylist - I’m on my feet most of the day.”
“I’m not really particularly stressed.”
Questions to ask (use to prompt the student if they miss these areas):
Why can menopause cause tummy problems?
Is there anything I could do with my diet to help with these symptoms?
I’ve heard that probiotics can help with gut problems, what are probiotics?
The student should refer you to the GP. If they do not, prompt them by asking:
“Do I need to do anything else or can I just try and manage this at home?”
Any criteria marked in bold must be met to pass this station.
